After much debate, I have decided to apply for mental disability. I have several mental illnesses which have gotten progressively worse. I realize that I lack the ability to function long-term as an adult should. I have been unable to keep a job due to my illness since 2005.

I have been in and out of therapy for the past 13 years do not remember some of the doctors I have seen. The disability application requires the names and addresses of the doctors who have treated you for the illness(es) which you are claiming on the application.

I don't know how to find the names of the doctors I don't remember. I was thinking it would be possible if I contacted the insurance providers I have had during the time I was in treatment but I don't know if that would work. Does anyone have any suggestions? I can't finish the application process until I provide the doctors information. Thanks in advance for any advice.

Some of your medical records would have followed you if you were referred to other doctors. Some doctors will ask to have medical records from other practitioners sent to them. Call your present doctor and see if they have any of your old records. The insurance company can certainly tell you who treated you. If you have forgotten the information then fill out the app. as completely as possible. I hope this helps.
