Hi
I'm 20 y/o, and circumcised (phimosis).
When I was about 9, I discovered a small
hard lump underneath the skin on the shaft
of my penis. A few years later, I
discovered another smaller one on the
underside. I would say the larger one is
about 3mm. It has not grown and can slide
under the skin a bit. Both seem to be at
the line where they made the foreskin cut,
and seemingly attached by what feels like
some sort of tissue ring underneath the
skin. I have assumed that these are cysts.
Does that sound correct?
Now, the more concerning bit. When I was
about 14, I found it harder to urinate.
Instead of going to the doctor (which I'm
terrified to do about this, fearing the
worst, and as a result of being teased at
school when I was younger for being the
only one circumcised), I thought I'd
examine myself. Now, I know I'll get told
off for being stupid, but I stuck a q-tip
with the end broken off down my urethra.
Nearly halfway down my penis, I found an
obstruction. Well, I decided to apply some
pressure, and it (excruciatingly) gave
way. It bled a little bit, and urination
was very painful for the next day. After
that, it was fine for several years. Now,
6 years later, it feels like it is coming
back. I'm really scared and know that I
can't ignore it again.
Does anyone know what is going on here? Am
I looking at a urethral cyst, or even
worse, anterior urethral cancer? I am
going to the doctor tomorrow for tiredness
problems, but I am honestly terrified
about mentioning any more. Any ideas on
what I have?

Do you have a sensation of inadequate
emptying when urinating?
Do you feel urgency to urinate?
Are you experiencing decreased stream and
postvoid dribbling?

Thanks for the response.
While it feels like I am fully emptying my
bladder, I do experience dribbling after
urination, sometimes several minutes after
(in very small amounts).
This past 2 weeks the problem has been
getting worse, with burning during
urination, and a weak start/end of flow.
I "bit the bullet" last week and saw a
doctor about it. He referred me to a
urologist, but I have to wait nearly 6
weeks for this appointment. However, I am
having a blood test this week b/c I also
complained of extreme tiredness lately,
and he specified a CRP test as part of it
(presumably to help test for cancer, among
other things).
Any more thoughts... I'm really quite
worried and he said there may be a
possibility of speeding up the appointment
if he "tacks on" something like cancer...
but I don't want to come across as a
hypochondriac either....
What to do???

Dear M&Ms,
today I got the blood results back from
the doctor.
Pretty much every count was normal.
Although C-Reactive protein was at 6.0
mg/L... I thought that this level should
be about zero?
My other symptoms are getting worse. My
prostate is definitely enlarged (although
the doctor seemed at first like he didn't
believe me) I think that symptoms such as
decreased urine flow, burning, and
difficulty and pain at passing anything
but the softest stool, combined with this
constant burning sensation from this area
are all pretty good indicators, not to
mention that a few times passing stool has
resulted in prostatic discharge
(presumably from pressure from the stool).
The doctor seems to believe me now, yet
perhaps not wanting to believe that a
non-sexually active 20-yr-old could have
(what I'm confident) is either prostatitis
or prostate cancer. It seems evident that
it's on the back of his mind though, as he
has nervously thrown around the word
cancer several times without me actually
suggesting it.
Anyways, that aside, he said he can do
nothing to speed up the 3-week wait to see
the urologist, but said that I might be
able to.
If the pain and symptoms continue to get
worse, should I be bugging the hospital
for an earlier appointment, or should I
bear it and wait the 3 weeks?

Having CRP less then 10 mg/L is considered
normal.
I guess your urologist will examine you
for possible lower urinary tract
obstruction, obstruction of bladder or
urethra (obstruction often leads to
urinary tract infection that can manifest
with frequent urge to urinate and a
painful, burning feeling in the area of
the bladder or urethra during urination;
it is not unusual to feel bad all
over—tired, shaky, washed out—and to
feel pain even when not urinating).
Do you feel need for urinating during
night?
Are you experiencing problems with your
bowel movements, too?

No problem about the reply time.
I saw a doctor again last Friday because
the problem was getting continually worse.
Thankfully the problem does not seem to
have worsened during the past week.
He called the urologist who could do
nothing to speed up the appt, but
hopefully this will not be necessary.
While I have not been experiencing an urge
to urinate at night, I experience a fair
bit of pain with bowel movements, and it
is hard to do so. Kinda like you might
expect with hemorrhoids, except that I'm
confident this is not the case. Urination
often becomes noticeably more
difficult/painful after a bowel movement
(presumably from pressure against the
prostate??). Urination is also very
painful/difficult up to several hours
after an ejaculation.
Oh well, I'll just have to wait and see
what the urologist says.

experience some burning during urination
following ejaculation.

I am often feeling like I need to have a
BM, and sometimes get this strange
cramping feeling in my rectum.
The burning I seem to be getting during
urination is much worse than one would
normally expect.
I am seeing the urologist next Wednesday,
so hopefully I will have some answers and
let you know.

I went to the urologist today. He did a
rectal examination, which was somewhat
excruciating as he felt and pushed around
the prostate
Afterwords, he said that the prostate felt
"fairly normal" but that I may have had a
bit of prostatitis. I suppose that would
fit in with the symptoms and the fact that
the problem has abated a little bit
lately...
He has now ordered an ultrasound and a
cystoscopy (eek!), but I kinda figured
that would happen anyway. He wants to
further investigate my bladder and
urethra.
Now more waiting...

I was surfing the web trying to see if I
had some sort of STD or something and come
across this forum. I as well have a lump
on the base of my penis. It doesnt really
hurt but it does look pretty swollen. I
dont have trouble urinating or anything
but I was curious if your symptom started
out like this. I have sex pretty often
and its usually with different women. I
was thinking it may be an ingrown hair, at
least I am hoping it is since thats no big
deal. Any insight?

I noticed the lumps when I was young, I
never really noticed any swelling, they
seem to be very-small-pea-sized cysts that
kinda move around under the skin.
You'd be better off asking your question
in the Men's Helath -> Penis Section of
this forum, as there are a lot of sexual
health questions there...

times.
I am meant to be going back in a few weeks
for another appointment with the
urologist. I just don't understand all my
problems and it frustrates me more that
they haven't found anything yet. I know
I'm not just imagining the pains, they are
really occurring, and it makes me feel
stupid that they have not found a cause
yet.
I just wish all these stupid problems
would go away

Epididymitis, an inflammation of
epididymis (part attached to each testicle
where sperm maturate), gives the symptoms
you were experiencing (painful urination,
painful bowel movements, testicular pain,
pain with ejaculation).
Enlarged prostate, prostatitis, urinary
tract infections, are risk factors for
epididymitis. Patients with urinary tract
infections or prostate infections, have
risk for bacteria to spread from the
infected site to the epididymis.

Hi Marija,
thanks so much for that info.
Unfortunately, I have come back to the US
(where my parents live) for 6 weeks, and
so I can't see the same urologist that I
have been seeing until september.
My symptoms have improved, but I still
experience painful urination and
testicular pain, to a lesser extent. I
will try and get tested for this condition
as soon as I get back. Prostatitis with
epididymitis now seems like a good
explanation. It would also explain the
referred pain in my back.
However, there is still the unresolved
issue of the lumps on my penis and the
apparent urethral blockage that seems to
have disappeared since the cystoscopy
(perhaps I did have a small stricture that
got displaced and not noticed during the
seemingly rapid "trip" to my bladder...
this still seems so odd to me...)
Do you know what needs to be done in order
to confirm that I have cysts or otherwise?
Does a biopsy need to be done? Or will the
urologist just know?

Actually, lower back pain and groin pain
are symptoms of epididymitis.
Urologist will know what needs to be
done.
I guess, the first thing the urologist
will perform will be urine tests and
prostate examination.
Have you noticed some urine
dicscoloration?
Have you experienced slightly increased
body temperature (mild fever)?
Have a great time back home!
