or do you think he is just being selfish?

Yes he could change if he wanted to. If
he cannot make it happen though without
meds than I would insist that he went on
them.
I don't waste my time on minute men, or
even anyone who lasts less than twenty
minutes. That is a quickie for me. No
point in taking off your clothes if it
isn't going to be worth while.
