Persistant headache

Hi,

I'm posting for my wife because she is unable to look at a computer screen. She is 40 y.o. and has had problems with headaches for the last couple of years.

The problem seems to have started when she got a new pair of glasses with a new prescription in June 2006. Whenever she wore the new glasses she would get bad headaches, describing the pain as being behind her eyes and it felt like her eye was "pulling". The optician told her to persevere until her eyes adjusted to them, but she didn't adjust and went back to her old glasses and the headaches disappeared.

New lenses were fitted into the glasses and also her original prescription was put into the new frames, but she was never able to wear the new glasses without experiencing the headaches.

She suffered from bad colds in October and again in December 2006. After both she suffered from headaches, even with her old glasses and was prescribed anti-biotics as the GP thought she had sinusitus. The first time the headache went. The second time it didn't.

She was given various anti-biotics as the doctors treated her for sinusitus, but the headaches never went. She also had numerous eye tests, which all conclude that her eyes are healthy for somebody with high myopia (short-sightedness). One of the opticians diagnosed her as needing a slight prism in her lenses. The prism lenses were put in the second and a third pair of glasses frames.

Meanwhile, Ears Nose and Throat at the local NHS Hospital stuck a camera up her nose, found nothing and prescribed another batch of anti-biotics, diclofenac and nasal spray that did nothing. Also, the GP was prescribing higher and higher pain killers from paracetemal with codiene, to co-prodamol, to tramadol. She was also signed off of work.

I was able to get her on my private health insurance through my company and got her an appointment with a neurologist in June. The neurologist prescribed amitriptyline and sent her for an MRI scan, which showed nothing abnormal.

The Amitriptyline began to work and she weened herself off of the painkillers. An appointment for a C/T scan finally came through in July, and this also showed nothing when she finally got the results in August.

After upping the dosage of the amitrityline as prescribed by the neurologist, she began to drop the dosage due to the side effects of the amitriptyline - being tired all the time.

When she saw the neurologist again in October she was down to 5mg of amitrypiline. The pain hadn't gone completely, but was manageable. The neurologist said she could try coming off the amityptilene as he could not imagine the small dose was doing a lot.

The headaches got worse after she stopped the amitriptyline and she started back on it. The GP has also now put her back on Diclofenac and also prescribed a nasal spray (going back to the sinus theory again).

She has also been seeing a chiropractor for the last month.

Originally she described the pain as behind, above, and between the eyes in the middle of her forehead. The pain now is often described as a line going up over the top of her head, just to the right of the center of her head. She even has trouble laying down to sleep as the pillow puts pressure on her head.

She also has problems reading or looking at a computer screen, and if she is having a "good" day, doing either of these will quickly turn it into a bad one. She also takes off her glasses often as this helps to relieve the pain a bit.

Nothing seems to have helped and I was wondering if anbody had any suggestions or ideas of where to go from here. Thanks.

Are her eyes sensitive to light, especially when headache is present?
Does this pain begin in the lower area of the neck and spreads upward on the side of the head?
Does your wife find the entire scalp and forehead painful?
Is the pain aching, burning, and throbbing, that is continuous, with intermittent, shocking, or shooting pain?

A touch, but no more than before she was ill.

No, it's a dull aching. Almost feels numb on the forehead, and she has a line that feels bruised that runs by the side of her right eye and over her head. It can feel like touching a bruised area when she lies down on a pillow or rests her head while sitting on the back of a sofa.

Does she get headaches only when looking at the computer or reading?
Do the headache is always preceded with eye pain?
Is she experiencing this headache as a period of attack or is a daily issue?

No, not only.

I can most definatley relate to the headaches! HAve suffered from them for well over 30 years. I did however have a cerebral anuresym 8 years ago, but still have migrane type headaches. Light sensitivity and noise are still a problem as well. I have been on every type of medication you can imagine. When your wife had an MRI did she also have an MRA? I can not forh te life of me remember what this is, but do know its an important test for ruling out anuresyms. I am currently taking 50 mg nortriptyline everyday, and this is currently working. I have been on this from my neurologist for almost a year now. I can deal with being tired a lot better than I can with pain. Has food allergy been ruled out? Does your wife consume products containing aspartame or sugar substitutes? Sometimes shellfish, cafffeine, etc, can cause headaches. MandMs knows her stuff when it comes to medical "stuff". She is very informative and helpful. Just thought I'd throw my 2 cents in too! Hope your wife finds some relief soon!

Thanks for your input. Food allergy hasn't been ruled out and hasn't even been mentioned by any of the doctors. The GP's seem to just want to prescribe pain killers and not investigate why the pain came on in the first place.

Find another physican, or if you can find a neurologist without a referral. yhey will get to the root of hte problem. GP.......give you and EDUCATED GUESS and pain meds. Please find another physican who will take this seriously..........headaches are a pain in the YOU KNOW WHAT! Hope she will find relief soon!

Sometimes glasses can put pressure on the bridge of the nose and this will cause a headache. If I wear my glasses and start a headache I have to take them off or the headache will escalate to the point of making me nauseous and really sick.

I think the allergy idea is worth looking into.

Another thought - if the sinuses are involved you can get a nasal bidet to rinse and flush the sinus cavity. This has been very successful for many people. It may be worth a try instead of more antibiotics.

Does she feel discomfort or a mild, one-sided burning sensation of the head before headache onset?
Is the eye pain accompanied with eye tearing and nasal congestion?

Eye pain and one sided tenderness of scalp can be signs of cluster headache.

I'm not sure she has it, but, is worthed to be discussed with her neurologist, because atypical presentations of cluster headache are more common in women and also, when cluster headache becomes a chronic form, usually, a continuous milder background headache additionally develops.

Hi,
She has complained of nasal congestion, but not eye tearing. Not sure of the burning sensation - she sometimes thinks the one side of her head is hotter than the other when the pain is present.

She also reports a "popping" sensation inside her head, on her bad side, somewhere around the side of her nose and her eye - not sure if it is a muscle or inside sinus, usually in the morning immediately after getting up. One or two "pops", after which there is a slight feeling of relief. She also says she will often have a better day if she has the pops in the morning.

Thanks to all for taking the time to respond. All feedback is greatly appreciated and has given us things to think about and consider. She is going to make another doctor's appointment on Monday.
