Hello -
Several months ago the frenulum below the
glans of my penis suddenly became
tightened and sore (mildly sore when
flaccid, and painful when erect). I'm 25
years old, circumcised, and have never had
sex yet in my life. I visited a
urologist, who recommended a frenuloplasty
to correct it - however I wanted to try to
get a few additional opinions before
consenting to surgery.
I have 2 questions I'm hoping for advice
with:
1) Along with the tightened frenulum, a
vein on the left side of the frenulum has
appeared where nothing was before (at the
same time that I noticed the tightening of
the frenulum). The vein is larger and
darker than any other on my penis (looks a
bit like bruising under the skin), and
begins under the frenulum and ends a about
1/2" away at my circumcision scar. I
showed this to the urologist, but he
brushed it off as nothing - however, since
it appeared simultaneously with the
tightened frenulum, it seems worth
considering. The extreme proximity to
where the frenulum would be cut is also a
concern to me.
2) In researching what a frenuloplasty
involves, I've learned that some doctors
do one or more simple horizontal cut(s)
across the frenulum, while others do a
more advanced "Z-plasty" that may minimize
scarring. If my doctor recommends the
"simple" method, should I request a
Z-plasty instead (or vice versa)? If a
Z-plasty is best and he is unwilling or
unable to do it - I'd be open to changing
doctors or even paying the full price of
the surgery to make sure it's done right.
Any advice on which to choose would be
appreciated.

photos showing the affected frenulum and
vein in case anyone is willing to view
them to provide more accurate guidance.
