He may have stretched his frenelum (the
arched piece of skin at the front directly
under the head) during sex which is a
common enough occurence. This usually
doesn't need to be treated medically. Just
needs to be kept clean and allowed heal
up.

My Fiance woke up this morning and noticed
two spots of blood in his underwear but
when he used the bathroom there was no
blood in his urine.
