Joint arthalgia and persistent ASO titer
Posted: 02-03-08 08:32am

I am new to this forum - below is my history - any thoughts would be appreciated:

I am 40 year old female - general good health

Sept 2007 Reactivated mono, ASO titer 416 treated with amox 10 days

Oct-Dec 2007 - joint pain in elbows - worsened each month, most painful in left elbow, fatigue continued

Dec 2007 - ASO titer 400, pain in elbows, wrists (left elbow considerable pain) general malaise - treated with cephalosporin

Jan 2008 - ASO 417 joint pain every day (elbows, wrists, knees), slight swelling to left elbow, pain in knees, ankles and feet when I get up

Each morning is difficult now, low grade fever 99 certain times of the day, continued fatigue; occasional discomfort in chest on left side); episodes of weakness in knees, cannot stand comfortably (like my knees are going to give out) - last 1-2 hours, then subsides

I have been to see a Rhumatologist, who gave me short term pain meds that I use sparingly, and now have an appt. with infectious disease dr. and for 2D echo; follow up with rheum. next week.

I was given RX Feldene for a short period, which made me feel great but was taken off that as rheum thought it was not right med for me, considering I had hiatal hernia in 1989?

I had strep throat in highschool, no other time that I am aware of

CRP and sed rate continue to come back normal

I have mediterranean anemia (thallas minor)

I now have a small lump almost to the inside of my left elbow (not protruding, just raised area of skin)

Has anyone had these symptoms?

Thanks

Are you experiencing morning stiffness of your joints?
Have you noticed that stiffness and pain become worse with weight-bearing activity?
Do you find it difficult to get up from a chair or go up or down stairs?
Is the joint pain (comes and goes) aching in nature?

I have morning stiffness, but contant soreness in my left arm.
I can't put any weight on my elbows, but I can still carry a bag, even though it hurts when I am done. I am having trouble holding onto things with my hands.
Yes to all of your questions.

Do you feel extreme, widespread pain in your body?
How do you know you have reactivated Mono?
Do you find your neck lymph nodes swollen?
Do you have a mild pharyngitis?
Do you have joint pain without swelling?

Your symptoms are typical ones for rheumatoid arthritis, and this disease, also, can cause an overall feeling of sickness and fatigue as well as weight loss and fever.
Rheumatoid arthritis gradually causes your joints to become deformed, decreasing joint mobility and range of motion.

It may be normal the ASO titer initially rises even after the eradication of infection.
The increase in titer is due to previous, recent infection and will eventually come down on its own with time (ASO titers are likely to be raised for 3- 6months).
