Well when I went for my first appt, they
gave me a bunch of forms and one was for
registering. As my ob-gyn's office is at
the hosp where I plan on delivering (and
where I was born!!!) it makes things
easier.
Pick a hosp close to you (with a good rep)
and call them and ask. They'll probably
send you forms.

I received that same form. It's suppose
to speed things up. I figure your doc
should be giving you all the info. Ask
:)

I never registered until a few months
before I delivered, but my doc told me
where to go and what to do, so I just
pretty much rely on him for that stuff.

They def. Give you a hospital form that
you are suppose to fill out. And they
tell you to fill it out and mail it back
in around 22 weeks. If your dotors office
gave you a goody bag at your first appt.
It would be with all of those papers.
Good luck
