when i urinate i feel pain on end of the the penis ,my underware is showing blood pls advice

You probably have a urethral inflammation. Bloody discharge inicates severe inflammation. Bloody urethral discharge is also present in other diseases (some of them very serious). You can go to an urologist immediately for an examination. Standard blood and urine analyses will be needed.
