Nontherapeutic effects and toxicities:
1. Hyperkalemia, more common when
potassium-sparing diuretics used as sole
diuretic agent.
2. Acute renal failure. The combination
of triamterene with indomethacin has been
reported to cause acute renal failure.
3. Kidney stones. Triamterene is poorly
soluble and may precipitate in the urine,
causing kidney stones.
Contraindications and precautions:
1. Oral k+ administration should be
disconitnued if triamterene is
administered, to prevent hyperkalemia.
2. Chronic renal insufficiency. Such
patients are vulnerable to hyperkalemia
and should rarely be treated with
potassium-sparing diuretics.
3. Liver disease. These patients may
have impaired metabolism of triamterene,
and dosing must be carefully adjusted.
