Non-union femur and humerus
case history of my father dr sant parkash
singh ms:
met a road traffic acident in the year
1992, october.
1) simple(non-comminuted and closed)
fracture shaft femur and patella.
2) simple(non-comminuted and closed)
fracture shaft humerus.
3 days later:
surgery done at goverment medical college
amritsar by dr jps chinna: plating of
femur with titanium plate –open surgery.
With wiring of the patella.
Fracture humerus -titanium plate fixed on
humerus.
Patient resumed slow activity on medical
advice within a month .
The femur plate broke.
Readmitted and in less than 3 months from
the a first surgery there was an
intra-medullary nail implanted with no
internal fixation screws.
No bone grafting done.
On x-ray examinations for next 7 years the
gap in the fractured ends of femur and the
humerus never filled . There were
radiological signs suggesting delayed
union and then non-union femur and
humerus.
2001: after 8 years the patient had a
small accident and felt that the
intra-medullary nail was slightly curved
due to impact of the accident. The were
no external injuries,
was advised exchange nailing with
internal fixation with screws with bone
grafting.
Which he underwent from Dr.Saluja and
Dr.Ravijit both ms orthopaedics at
silveroaks hospital , mohali., india.
The surgeons kept a gap in the bone ends
which didn’t fill up on x-rays after a
month.
Was advised ‘dynamization’ .
Patient was depressed with the results so
far and didn’t undergo the
dynamization.
The screws of the intra-medulary nail
broke.
The nail also broke but at its superior
end.
Patient walks around with an elbow crutch
conducting is daily chores and surgical
practice.
The patellar wiring was uneventful.
The humerus: the humerus plate broke less
than a year back .
The fracture has visibly not united in all
these years(>10 yr) . Now patient has
difficulty conducting chores using his
right arm.
He has been using an elbow crutch in left
hand for walking support all these years
since 1993.
Was diagnosed as diabetic first time in
the post-op period in the 1992.
The post-op period in the 1st surgery:
developed breathlessness and was given
fursemide timely and possible pulm edema
reversed.
Post op period in the 2nd and 3rd surgery
was uneventful
the surgical wounds always healed well.
C-reactive protein done recently(2 months
back) was negative.
Patient and his family wish him to have a
union of his humerus and femur for a
better quality of life and less dependence
on attendants.
Please help! Afer 3 surgeries over 10
years we desperate for a doctor who thinks
he/she can help
