I'll try to be concise.
Patient has episodes of drowsiness,
fatigue, disorientation, stumbling, vision
changes, and slurred speech. Mood
changes, ie. Fits of anger. Memory
loss. Falls asleep a lot at unusual
times. Difficulty swallowing. Swollen
thyroid. Loss of muscle function in face,
noticeable graying of the face. Eyelid
lag. Sixty-four pound weight loss.
Patient does not remember episodes.
Mri, blood testing, and ‘overnight’
urine test gave no results. Normal blood
sugar.
Low sodium was found. Thyroid sonogram
normal. Head ct normal. Pituitary
problem diagnosed initially at time of
episode. Initial testing during an
episode pointed to thyroid disorder, but
follow-up visit was not during an episode
and did not show the same results.
Thyroid history on father's side.
Other details:
patient is 34 yrs old. 6’8, 255.
Patient has history of substance abuse,
alcoholism. Current cigarette smoker, 15
a day.
Patient is currently taking prescriptions
of effexor xr 75mg 3x, xanex 1mg 6 x a
day, lisinopril 10mg 1 x day, and lunestra
1 x 3mg.
