can someone tell me the proper terminology
to use when requesting a copy of the
events that transpired while I was in
surgery?

I simply stated in writing that I wanted a
"full copy of all records from [date] to
[date] pertaining to [hysterectomy],
including all written records, graphs,
charts, photographs and any other records
available".
Very often you have to pay an
administration fee.
I didn't have to because mine pertained to
my making a complaint to the hospital and
they were required lawfully.
If you have any questions or complaints
and don't feel that you can ask you
surgeon, ask for a "patient liasion
advisor" at your hospital.
