I'm starting on the 5th day (thursday)
instead of sunday (which is labelled on
the pack). For the first pill do I take
the thursday pill or the sunday one?

If you are starting a brand new pack you
always start with 1st day sunday even if
you actully start on a thursday. In my
packes they come with a stick that you can
but on it to change the days so it matches
your schedule.
