Dear Doctor,
I am a 21 year old heterosexual male. I
am uncircumcised. Recent symptoms that I
have experienced have made me wonder about
my past sexual encounters and the
possibility of HIV. In November 2005 I
had protected sex with an escort in
Washington, DC (It cost $300 per hour). I
remember she applied some sort of
lubricant to the condom. I don’t know
if it was water based or petroleum based.
In any case, the vaginal sex lasted no
more than 10 minutes and the condom
performed well. It did not break. I did
not ejaculate either. About 15 months
later (February 2007 – in the thick of
winter), I began experiencing some
symptoms. I developed a rash that at first
had a burning sensation to it. The rash
consisted of patches of tiny red bumps
(barely raised) on the inner side of my
biceps and on the inner side of my thighs.
Thus, it concentrated on the upper side
of my extremities. I was on an overseas
academic trip and my busy schedule simply
did not allow me to see a doctor. I
decided to run into a drug store in
Germany to buy some over the counter
cortisone cream for the rash. The burning
feeling quickly went away with the cream
and the rash went away for the most part.
However, a little bit of the rash remained
for about a week, but it did not burn or
itch. By the end of that week, I began
feeling a fatigue that lasted for about 3
days and came and went in the course of
one day. An increased heart rate was
associated with this fatigue. I also had
a fever that lasted only one morning. I
also had a sore throat and a few chills
that lasted for a couple of those days.
Some mild sinus symptoms were also
experienced. When I got back home from my
trip, these symptoms were largely gone so
I dismissed them. But now, I have been
searching for what initial HIV symptoms
might look like and I have been terrified
with many things I have read. I have
abstained from any sexual encounter since
my last one in Nov. 2005. Prior to that,
my only other sexual encounter consisted
of a woman performing unprotected oral sex
on me. Thus, I do not consider myself
sexually active. The symptoms I have
described have been the only health issues
I have had since the sexual act on Nov.
2005.
Could these symptoms have been associated
with HIV?? Should I get tested? Do
initial HIV symptoms develop as late as 15
months after a possible exposure? Please
enlighten me on this. Thank you in
advance, I find it very kind of you.

HIV symptoms may take a very long time to
show up. Many people do not know that
they have HIV for quite some time, making
it even more dangerous as they may
unknowingly infect other people. Many
people have no symptoms at all when they
test positive. Some people have HIV
without any symptoms at all for ten years
or more.
The only way to know if you have HIV is to
get tested. Symptoms alone are never a
certainty for HIV or anything else.
Condoms do not offer 100% protection, so
every sexual encounter is a risk, but
sexual encounters with escorts are usually
even bigger risks. There is no harm in
getting tested just in case. HIV takes a
little while to show up on tests, but if
you are talking about something from 2005,
then that is more than enough time. That
means that you can go right in and get
tested.
Talk to your doctor about all of your
symptoms and see if he or she thinks it
might be wise to test for anything else
too. If you do not have insurance or a
regular doctor, you can always go to a
clinic. Just don't be afraid to talk to
whomever you do see.
